Sažetak
Aim of the study is to evaluate results of operative treatment of posterior acetabular wall fractures and to identify factors responsible for unsatisfactory postoperative results. From 1993 to 2003 total of 137 patients were operatively treated at our Department for posterior acetabular wall fracture. 87 patients were available for follow up and final evaluation was performed on average 5 years after the surgery (range 2-10 y). Preoperatively in all patients CT and standard set of X-rays (standar d PA and two oblique views according to Judet) were taken. Postoperatively standard set of X-rays in all patients and CT scans in selected patients were taken. In all patients functional evaluation according to modified Matta questionnaire was performed. In 40 out of 87 patients functional result was excellent, in 26 results was very good, good and satisfactory result was in 10 patients and unsatisfactory result was seen in 11 patients (12%). There was strong correlation between postoperative X-rays and functional outcome. Unsatisfactory results were seen in cases with poor fragment realignment. Factors influencing unsatisfactory outcome after operative treatment of posterior acetabular wall fracture include higher patient's age, longer time between injury and surgery, remaining gap after fragment reposition, osteonecrosis and joint line decrease.In cases were anatomic fragment reposition can be achieved postoperative outcome will be good to excellent.
