Sažetak
In addition to the usual activities and standard practice during the past two years, the Internal Medicine Office in Cres has used telemedical consultations in the process of diagnosing and treating acute and chronic diseases in 80 patients. A total of 278 teleconsultations has been arranged with specialists in different fields of medicine, composing the "Virtual Polyclinic" project. Telemedical equipment included Pentium III based computers, a high resolution digital camera, the ultrasound unit, standard and tele-ECG devices, scanner and printers. We also used data from a 24-hour blood pressure monitoring device (Holter) and the laboratory unit. The latest versions of Issa and Pharos software allowed storing and transmission of virtual patients' records over an ISDN line, including different file types, usually series of high-resolution images accompanied by textual data. Both, diagnostic study and cost-effectiveness analysis which are currently in progress, evaluate this telemedical system in the environment of a specialist care unit, practicing internal medicine either as a specialist care provider or in consultations with other specialists. Preliminary results indicate a high level of diagnostic accuracy and quality of treatment, as well as a good cost-benefit ratio in the period of two years.We invested 5 300 $ and gained back more than 6000$ before the end of one year of rutine work.We saved 49$ per capita in the initial tretement of our patients and 36$ per capita for in the follow up.
