In our group of 20 clinically nonfunctioning pituitary adenomas (NFPA) LH and FSH were measured after TRH stimulation in vivo. 9/20 (45%) patients demonstrated significant LH responses (more than 30 %) and 3/20 (15%) significant FSH responses. 10/20 (50%) responded with at least one hormone. TRH caused minimal or no response in the control group. After the surgery adenomas were cultivated and the quantity of LH and FSH was measured in cell culture media. 16/20 (80%) tumours secreted LH and the same number of tumours secreted FSH into culture media. 18/20 (90%) tumours secreted at least one gonadotrophin in vitro. In 15/20 (75%) patients LH, FSH or hCG were detected immunohistochemically. A significant correlation was demonstrated between LH secretion into culture media and the maximal percentage response to TRH in vivo (r=0.69, P<0.05) and also between LH secretion and nAUC (r=0.73, P<0.02). A significant correlation was obtained between FSH secretion in vitro and nAUC after TRH administration (r=0.76, P<0.01). We conclude that most NFPA release gonadotrophins in vitro and that the responses of LH and FSH following TRH administration in vivo can identify those tumours which will release gonadotrophins in vitro. These tumours are presumed to originate from gonadotrophs.
