Objective: It is still opened question how CS affects renal hemodynamics and whether it influencing clinical course increases risk of ESRD. Our aim was to analyse clinical charatersitics of patients with PGN regarding their smoking habits. Patients and Methods: Clinical characteristics of 369 patients with biopsy proven PGN were analysed (membranous GN 70, mesangioproliferative 111, IgA-N 85, focal segmental glomeruloscelrosis (FSG) 65, membranoproliferative 38). There were 160 (43%) habitual smokers (S) and 209 (57%) non smokres (Non-S). Arterial hypertension (AH) was defined as RR >= 140/90 mmHg, nephrotic proteinuria (NP) as >=3.5 g/dU and azothaemia as serum creatinine (C) >= 125 µ mol/l (men) and >=115 µ ol/l (women). Results: There were no differences between S and Non-S in age and duration of disease prior the renal biopsy (21.4± 10.2 months vs. 22.2± 9.3 ; p > 0.05). There were more men in S (122 (76%) vs. 119 (57%) ; p < 0.05). S had no more frequently AH, NP and asothaemia either basally or at the end of follow-up (FU), neither in the whole group nor in the particular subtypes of PGN (p > 0.05). However, higher values of C were observed at the end of FU in FSG (145± 36 vs. 120± 25 ; p < 0.05). Conclusion: We failed to find any significant influence of CS on clinical course of PGN. This could be explained by the development of tolerance to the renal effects of nicotine. Influence of smoking should be very probably evaluated considernig gender and using more sensitive methods analysing data of each clinicopathological subtype separately.
