Sažetak
This study shows the course of treatment and prothetic rehabilitation of a young girl who was injured in the traffic accident. She was transferred to our Department from another institution more than four hours after accident. At admittance she was GCS 10. Intraorraly the upper jaw was mobile. The fracture of occipital bone and brain contusion was diagosed. The girl was intubated in ER and transfer to operating room. Intraorally there was found the avulsion of the hole of premaxilla with all the teeth knocked out. The bone segment was in the oral cavity and it didn’ t have any real contacts with gingiva or the periost. Inside the buccal mucous membrane a part of alveolar ridge of upper jaw with an germ of the permanent canine tooth and the germ of the first premolar were found. The segments of maxilla and teeth are washed in the saline solution, followed with the repositioning and fixation of maxilla with two mini plates. During the surgery, efforts was made to fix the germ of the two permanent teeth by an orthodontic appliance. At the first postoperative checkup it was found that a reinplanted left premolar was missing. Although more than six hours have passed from the injury till the reinplantation, the remaining tooth, the bone and the alveoli are vital, which can be seen later at the postoperative orthopanograms and clinical findings. Six months after the injury, at the removal of fixation materials, a normal bone structure could be seen. An orthodontic prosthetic rehabilitation was performed. Our case proved that primary reconstruction was definitely worth trying, even when more than six hours have passed after the bone avulsion. The repositioning and bone fixation of the fractured fragments are certainly the best solution, for the esthetic and functional point of view.
