Coeliac disease (CD) is a life-long intolerance to the alcohol-soluble fraction of gluten. Serology markers used as a screening tool are: antigliadin IgA and IgG antibodies, endomysial IgA antibodies and tissue transglutaminase IgA antibodies which have the highest specificity for CD. The aim of this study was to compare two methods for the quantitative measurement of IgA antibodies directed to tTG, enzyme-linked immunosorbent assay (ELISA), Eurospital, Trieste, Italy, and fluoroenzymeimmunoassay (FEIA), Pharmacia Diagnostics, Uppsala, Sweden. The study included 47 children who attended Children’ s hospital Zagreb for suspected CD (M/F=22/25, age 2 months– 28 years). Diagnostic specificity, sensitivity, positive and negative predictive values, ROC analysis and correlation coefficient were calculated for both methods. Duodenal biopsy was performed on all patients and was used for accurate diagnosis. Specificity, sensitivity, positive and negative predictive value: ELISA 100% for all parameters, FEIA 100%, 91%, 100%, 93%, respectively. ROC analysis showed higher discriminating power of ELISA (AUC=1.00) versus FEIA (AUC=0.998) at a cut off point recommended by manufacturer ; 5.6 AU/ml for ELISA and 7.0 IU/ ml for FEIA. Correlation coefficient was 0.802. The results indicated the same specificity and positive predictive value, with higher sensitivity and better negative predictive value of ELISA. ROC analysis showed high discriminating power (ELISA, AUC=1.00 ; FEIA, AUC=0.998) for both methods and that they could be reliably used in detection of IgA antibodies to tTG.
