Sažetak
The 48 years old man was referred to our hospital for neurosurgical therapy of failed back syndrome. Lumbar discetomy was done to the patient in another hospital 24 years ago. He was reoperated and another nine operations on lumbar spine were done during fourteen years from that period. Because of instability lumbar spine fusion was done four years before he was transferred to our hospital. The pain bilateral but mainly in left leg, described as burning, tearing and continuous, was indication for intratecal implantation of morphine pump. After one year the morphine pump was no longer effective. The DREZ operation was proposed to the patient and was done however without substantial diminished of pain. During all this period the patient was on high dose of various analgesics. Bilateral cingulotomy was proposed to the patient considering psychiatry exam. The stereotactic bilateral cingulotomy was done on May 2002. Two and a half years after bilateral cingulotomy, the patient is still without pain and he does not take any analgesic medication. The only noticed side effect was augmentation of libido that lasts for one year after the operation.
