Sažetak
Gastrointestinal bleeding is one of the most often and life threatening complications in patients with liver cirrhosis. We aimed to asses the impact of basic coagulation parameters values on variceal and non-variceal bleeding in cirrhotic patients. Prospectively, we collected and analysed 176 pts. with liver cirrhosis who were hospitalised at the Department of Gastroenterology. Laboratory values of thrombocytes, prothrombine time and fibrinogen concentration were analysed upon admission. Pts. were divided into two groups ; group A (N=88) ; GI bleeding pts ; group (N=88): pts. without GI bleeding, hospitalised because of other cirrhotic complications. Group A was divided in two subgroups: A1-pts.with bleedinng from esophageal varices (N=44) ; group A2-pts. bleeding from other parts of GI tract(N=44). The bleeding pts.(group A)had significantly lower values of observed coagulation parameters compared to non-bleeding pts (group B). Variceal bleeding pts(subgroup A1) did not differ from non- bleeding pts(group A2).In any analyzed parameters, while non variceal bleeding pts.showed significantly lower values. Non variceal bleeding patients had significantly lower thrombocyte values (p=0.005) and lower fibrinogen concentration(p=0.04) comparing to variceal bleeding patients (subgroup A1). Thrombocyte values, prothrombin time and fibrinogen concentration were lower in cirrhotic patients who were bleeding from GI tract, compared to "non- bleeding" cirrothic pts. Interestingly, only pts. with non-variceal bleeding showed significantly lower values of analysed parameters compared to non-bleeding cirrhotic patients.
