Sažetak
Introduction: Image methods (ultrasound-US, computerized tomography-CT and magnetic resonance-MR) have ensured the visualization of intra-abdominal tumors (of liver. pancreas, kidneys, adrenal glands and retroperitoneal nodes), which are very rarely approachable in palpation, and consequently in guided aspiration. Objective: (i) To analyse the adequacy of cytologic fine needle aspirated from the changed intra-abdominal organs and retroperitoneal nodes ; (ii) the occurrence rate of malignant lesions ; (iii) the occurrence rate of non-epithelial malignant lesions ; and (iv) the possibility for categorization of non-epithelial tumors by cytomorphology and additional technologies from cytologic specimens. Methods: 1333 intra-abdominal aspirates were examined: liver (653), pancreas (272), kidneys (67), adrenal glands (7) and retroperitoneal and mesenteric nodes (313). Fine needle aspiration was performed with the CHIBA needle. The preparations were ana lysed in standard stained smears by the May-Grunwald-Giemsa method, by determination of cellular markers by immunocytochemical analysis on smears and flow cytometry, cytochemicaL cytogenetic andlor molecular analyses (PCR). Results: In 1273 patients fine-needle aspiration was performed with the US controL in 10 patients with CT, and in only 36 patients without image control. We obtained too little material in 87 patients (6.5%). Malignant cells were found in 848 cases (64%) of all aspirates. In 199 aspirates non-epithelial tumors were found (35 sarcomas, 12 melanomas, nine embryonal tumors and 143 malignant lymphomas). Based on cytomorphology, completed with immunocytochemical markers in the sarcoma group, the diagnosis of gastrointestinal stromal tumor (GIST) was set up in three, rhabdomyosarcoma in four, angiosarcoma in one, schwannoma in three, liposarcoma in two, leiomyosarcoma in three, fibrosarcoma in six patients, while others could be diagnosed with sarcomas without precise differentiation. In 76 patients the malignant lymphoma diagnosis was set up for the first time. In newly detected lymphomas the cytologic diagnosis was completed, except for immunocytochemical analysis on aspirated smears or by flow cytometry, cytogenetically and/or molecularly (PCR) or patho-histologically on the exdsed node. Out of 41 patients with previously diagnosed malignant lymphoma, the abdominal relapses of the disease was confirmed in 26, tumor of another genesis was found in six patients, while in the others non-malignant changes were detected. Conclusion: Cytodiagnostic fine-needle aspiration of intra-abdominal organ changes, completed with immunocytochemical markers on smears and flow cytometry, by cytogenetic and molecular analyses from the specimens obtained by cytologic aspiration, has been proved as a reliable method in diagnostics and subcategorization of non-epithelial tumors.
