Sažetak
Functional gastrointestinal disorders are defined as chronic recurrent disorders of gastrointestinal tract in which organic or structural lesions cannot be identified. Most functional gastrointestinal disorders have their origin in disregulation of brain-gut axis as increased motor reactivity to several stressors and decreased pain tresholds.Mechanisms of these diseases are not identifide completely, but most of them are developmental diseases, inflammatory diseases and consequence of stress. Functional gastrointestinal disorders can be classified as irritable bowel syndrom (IBS), esophageal motility disorders and non-ulcer dispepsia.Treating patients with functional disorders is difficult because this group of patients is looking for physician who will treat their problems seriously as an organic disease. Pts. with functional disorders need symptomatic therapy, but many of them also need psychopharmacotherapy and psychological treatment.Mostly used drugs are antidepressants like tricyclic antidepressants selective serotonin reuptake inhibitors, novel antidepressants and anxiolityc like benzodiazepines.There some studies that have shown superiority of psychological treatment to conventional medical therapy. Future research in functional gastrointestinal disorders should be addressed at integrating the biological with psychosocial changes and at understanding better bi-directional communication between gut and CNS.
