Sažetak
Although prescription of antibiotics out of hospitals is significantly higher, hospital use of antibiotics is significant promotor of antibacterial resistance, financial burden and associated with adverse events which might prolong hospitalisation and further increase costs. Antibiotic policy in hospitals in Croatia is generally restrictive. Restrictive part represent process of registration, enlisting antibiotics on positive list of Croatian Institute for Health Insurance, hospital lists which usually have list of 'reserve' antibiotics. The most important and efficaceous restrictive measure is limited budget on every ward. Not restrictive measures are publishing of treatment guidlines – some are published and might be considered general, other are limited to single hospital. Treatment guidlines are adopted from medical literature and adapted for particular hospital. However, effective audit is usually missing. Continuous education is voluntary although it showed some effects in the past. Infectiuos diseases specialists are more and more consulted. Infection control measures resulted in dicreased number of nosocomial infections, which further decreased antibiotic consumption of some antimicrobials.
