Sažetak
Asthma is a chronic disease, representing he fourth leading cause of limitation of normal dialy activities with significant negative impact on quality of life. it can be controlled by preventive measures and effective drugs, preferably inhaled corticoids and beta2-agonists. According to guided self-menagement, proposed by GINA guidelines, daily dosage should correlate with severity of asthma symptoms and peak expiratory flow rate. Recording these data in ˝asthma diary˝ helps physician to gain a better insight in patient's condition and adjust therapy. Unfortunately, many authors reported poor patient´s compliance. The aim of our study was to use wide-spread GSM SMS and Internet Web technology in improving compliance and long-term control of asthma, creating of electronic, wireless, mobile asthma diary, which continuously records recieved SMS messages containing PEF results, measured by patients. Telemedicine system was consisted of GSM mobile phone, connected via serial link to the central computer. SMS messages were processed and stored in MS Access database by resident Visual Basic 6.0 routine. Statistics and graphic visualization of patient's PEF data were created on-demand using Web interface, MS ASP and calls to dynamic-linked library routine developed in MS Visual Basic 6.0. System is developed, programmed and tested during six months. It became active since the beginning of year 2004. During first week, thirty-two patients registered the service, recieved password and calculated their predicted PEF. More then half of patients checked their submitted data from home or work via Web interface. During follow-up examinations, physicians visited recieved online statistical and graphic report. All patients showed increased compliance. Not one SMS message was lost. The system was also used for simple, 2-way communication between patients and asthma specialists. It has potential of beins developed in National Asthma registry. Because of system's design, multilingual Web interface and support of ˝Croatian telecomm˝, this telemedicine service is accesible and opened to international users, free of charge.
