BACKGROUND Oral allergy syndrome (OAS) to fruits and vegetables often coexists with pollen allergy, and draws attention to the problem of false negative results of skin prick testing (SPT) with commercial allergen extracts. Apple is the most frequent cause of cross reactions in patients sensitive to birch pollen. Major apple allergen, 18-kDa protein, is responsible for majority of cross reactions, although different minor allergens (31-kDa, 50-kDa, etc.) may also react in some patients. The amounts of apple allergens vary considerably in different strains of apples, and are influenced by ripening and storage conditions. In present pilot study we performed a SPT with fresh and ripe apples of five different strains, in comparison with SPT with commercial apple allergen, and followed by oral challenge with the same apples. METHODS Three patients sensitive to birch pollen with a history of OAS were tested with one fresh and one ripe apple of five different strains (Granny Smith, Jonagold, Gloster, Idared, and Golden Delicious). Total of 10 SPTs were performed with a drop of juice squeezed from each apple directly to the forearm. All tests were done in duplicate, and the mean value of wheal diameters was used. Patients were also tested with commercial SPT apple allergen and RAST to apple. Oral challenge with a small piece of each apple used for SPT was performed afterwards. Three nonatopic patients served as a control group. All patients signed informed consents. RESULTS Ripe apples showed greater wheal diameters in SPT in comparison to fresh ones in all five different strains (mean wheal diameters: Granny Smith – 5.50 ; 4.58, Jonagold – 5.92 ; 4.58, Gloster – 5.42 ; 4.25, Idared – 5.58 ; 4.83, and Golden Delicious – 6.17 ; 4.08). The difference was statistically significant (t-value = 4.36, p < 0.01). At the same time, SPTs with commercial apple allergen were negative in all patients. Specific IgE antibodies to apple were detected only in one patient (RAST class 1 ; 0.50 kU/L). Severity of reactions in oral challenge tests was greater with ripe apples, which was in agreement with SPT. Controls were negative in all tests. CONCLUSIONS SPT with fresh apples is a useful tool in clinical practice, which can predict severity of OAS in birch sensitive patients. Although not standardized, it reflects a real situation due to lack of loss of its allergen content during preparation procedures. Confirmation on a larger sample is needed.
