This study examined a group of workers who started shiftwork for the first time at the age of 20 years and stayed on shifts for seven years. The purpose of the study was to find out whether various indices of shiftwork tolerance changed over this early period of exposure to shiftwork. Subjects (n=86) worked in a rapid forward rotating three-shift system with shift changes at 6, 14 and 22 hours. They were examined before entering shiftwork and followed up after 1.5, 3.5, 5.5 and 7.5 years of shiftwork experience. The following measures were recorded before subjects started shiftwork: Sleep Quality, EPI Neuroticism and measures of poor health (including Generally Poor Health, Musculo-Skeletal Complaints, Respiratory Complaints, and Psychosomatic-Digestive Complaints). In addition to these measures indices of Night- and Morning-Shift Tolerance and sleep duration on different days within shiftwork cycle were recorded at four follow-ups. Differences between repeated measurements were tested by means of MANOVA. All the measures of poor health and Neuroticism increased after subjects started shiftwork as compared to pre-shiftwork level. Within the period of shiftwork exposure Psychosomatic-Digestive Complaints showed additional increase while the score changes of the Generally Poor Health, Musculo-Skeletal and Respiratory Complaints were insignificant. After the initial increase from the pre-shiftwork level the Neuroticism scores marked a decrease within the period of shiftwork exposure. Sleep Quality scores did not change significantly either from the pre-shiftwork level or within the period of exposure to shiftwork. The measures of tolerance to a specific shift showed the opposite trend of change over the seven-year period: Morning Shift Tolerance was increasing while Night Shift Tolerance decreasing. The short sleep duration on morning and night shift days did not change significantly over the seven-year period. However, the sleep duration on both the afternoon shift days and days off were decreasing over that period. The results show that the greatest decrease in subjective health and increase in neuroticism takes place at he 1.5 years of shiftwork experience with respect to the level before entering shiftwork. During the period of exposure the extent and direction of change in other tolerance indices depend on the measure concerned.
