INTRODUCTION: Iron (sFe) participates in variety of vital processes in the body varying from cellular oxidative mechanisms to the transport and delivery of oxigen to body cells. The remaining body iron is present in the flavoproteins, the ion sulfur proteins, as well as storage iron-feritin and transport iron transferin. THE AIM of study was to investigate the relationship between acute inflamation and serum iron status. MATERIALS AND METHODS: We studied 57 healthy individuals (group H) and 63 patients with acute inflamation (group AI). CRP >8 was considered to indicate inflamation. sFe, C-reactive protein (CRP) and ferritin (sF) were determined by assays Olympus Diagnostica GmbH, Ireland (sFe – photometric colour test, CRP and sF immunoturbidimetric test), Fesat was calculated (Fe/TIBC)×100, using Olympus 2700 on- analyzer software. RESULTES: Median values of all analytes showed significant between-group differences (Mann-Whitney, p<0.001). In group AI, correlation were observed between sFe and Fesat (r=0.904), Fe and sF (r=0.311), Fesat and sF (r=-0.592) and CRP and sF (r= -0.311) (Spearman, p<0.001). In group AI, clinical accuracy expressed by ROC analysis showed AUC and cut-off values for sFe (0.994 ; ≤ 8.4 μ mol/L), CRP (1.0 ; > 8.7 mg/L), sF (0.828 ; > 158.8 μ g/L) and Fesat (0.953 ; ≤ 23.5%) with specificity 100%, 100%, 86.0%, 91.2%, respectively and sensitivity 100%, 93.7%, 68.3%, 90.5%, respectively. Conclusion: An inverse relation of CRP and sFe concentrations, and of CRP and Fesat values in acute inflammation not accompanied by anaemia (hemoglobin within the reference range) appears to suggest that a low serum iron level need not always reflect iron deficiency in the body.
