Sažetak
Aims. To determine the frequency of tolerance development to nitroglycerin infusion during the 72-hour period of acute myocardial infarction or unstable angina pectoris treatment. Methods. Tolerance development to nitroglycerin infusion was determined by measuring changes in blood pressure following the periodical discontinuation of infusion every 12 hours over the period of 72 hours. The study included 60 patients with acute myocardial infarction or unstable angina. Loss of 10% increase in mean arterial blood pressure as a response to 30-minute interruption of nitroglycerin infusion was considered as a sign of tolerance. Results. The number of patients who developed tolerance increased progressively during the first 24 hours (p<0.01). In most patients tolerance appeared during the first 36 hours of NG infusion (p<0.01). After 12, 24, 36, 48, 60 and 72 hours of infusion, tolerance was registered in 57%, 73%, 82%, 83%, 87% and 92% of the patients, respectively. Conclusions. As 82% of the patients developed tolerance during the firts 36 hours, interruption of infusion seems to be indicated during further nitrate treatment. A new model for tolerance investigation is suggested.
