We analyzed a total of 840 patients (480 male and 360 female) with nonmalignant lymphadenopathy aged 7-54 years. In 45 (4.8%) of these patients, lymphadenopathy was caused by cat-scratch disease (CSD). The diagnostic of CSD is not easy and today still not exact with available routine tests. In our laboratory, we tried to perform visualization with the Warthin-Starry stain, but without success. Cultivation on endotracheal Bartonella henselae culture is still not routinely used in our laboratory. Therefore, CSD was diagnosed in our patients based on epidemiological data, clinical features, and cytological finding of fine needle biopsy, which was given a special attention. By monitoring the dynamics of cytological changes on lymph nodes of our patients, we have noticed that in the majority of them a nonspecific lymphoid hyperplasia was detected in the first ten days with pronounced stimulation of mononuclear phagocyte system, followed by granulomatous reaction and finally purulent inflammation often accompanied by necrosis. In the majority of patients that received therapy (macrolides or tetracyclines) at the beginning of disease, purulent inflammation did not occur. Enlarged lymph nodes persisted from 14 days to 3 months.
