Sažetak
We presented a 5-year-old boy an atypical clinical picture of dermatomyositis. First symptoms of the disese appeared of 1 year.Livid macular eash distributed on the face, inner aspects of his arm, buttocks and lateral acpest of this trunk. He was suspected to have Rothmund-Thomson syndrome although typical poikiloderma was not present.We started treatment with prenisolone in the dose of 2mg/kg/day and methotrexate 0.5 mg/kg/week, Since he had severe osteoporosis abd soft tissue calcifications diltiazem, alendronate, vitamin D and calcium supplement was prescribed.Physical therapy was performed daily. The response to this therapy was fair regarding muscle strenght, his osteoporosis is less severe but soft tissue calcifications are still impressive. Clinical course of the disease was complicated by intestinal perforation and peritonitis, probably caused by vasculitis as well as by respiratory symptoms due to respiratory muscle weakness.
