Sažetak
Modern esthetic restorative dentistry is based on adhesive techniques. Adhesive dentistry demonstrated great improvement from the time it was introduced. The adhesive systems enable bonding to enamel, dentin, amalgam, metal and porcelain thus giving the practitioner great possibilities for various dental treatments and application procedures. The major reasons for using dentin-bonding agents are to promote the retention of the restoration with minimal loss of tooth structure, to reduce microleakage, marginal staining and recurrent caries and to effectively distribute occlusal stress. There are numerous indications for adhesive restorative procedures which simplify every day clinical work. Some simple prosthetic and orthodontic cases can be easily solved conservatively such as insertion and adhesive cementation of esthetic intracanal posts, composite restorations on heavily damaged teeth, restoration of fractured or discolored teeth with composite veneers, diastema closure etc. The main limitation of adhesive procedure is technique sensitivity which often causes postoperative problems because of inappropriate clinical application. According to bacterial microleakage concept, pulpal reactions observed after cavity filling placement are mainly due to the passage of bacteria between dentinal walls and filling material toward the pulp rather than remaining dentin thickness. It leads to conclusion that the restoration of choice is an adhesive restoration which minimizes micro or nanoleakage by enabling formation of a homogenous block with dental structures.
