Sažetak
Molecular techniques have improved health care by the remarkable sensitivity and specificity in detection of viral pathogens in various body fluids and tissues. Both target and signal-amplification methods are currently used in most virology laboratories. With improved and automated nucleic acid isolation techniques and real-time detection of the target sequence a new generation of assays for clinically most important viral pathogens have been developed. These improvements generate the results in a very short turnaround time and enable the accurate quantitation of viral target. The advantages of molecular techniques are extremely useful for the diagnosis of viral infections in immunocompromised patients, who are not able to give an appropriate serologic response and in whom we need a quick diagnosis in order to start pre-emptive therapy. Molecular tests are now being widely used to monitor viremia in HIV+ patients. Viral load is used as a tool to decide when to treat and how to identify treatment failures. Viral resistance tests have also been accepted as a standard of care in developed countries. Information about the mutational patters in the HIV Protease and Reverse Transcriptase genes is obtained and can be translated into an estimation about the activity of the antiretroviral drugs In transplant recipients longitudinal monitoring of CMV and EBV load in perpheral blood is increasingly recognised as a valuable diagnostic tool in prediction, diagnosis and therapeutic management of CMV and EBV induced postransplantation diseases.
