Sažetak
Background: New psoriasis therapies offer great hope for the people who suffer from psoriasis and other chronic inflammatory skin diseases. Psoriasis and psoriatic arthritis are the diseases that can be treated with biological therapy which targets either the interaction between an antigen-presenting cell and the T-cell, or cytokines such as tumor necrosis factor alpha (TNF-α ) that are presumed to be involved in the pathogenesis of psoriasis vulgaris and psoriatic arthritis. Objective: Current biologic therapies now seem to hold promise of potentially excellent effect: etanercept ; efalizumab ; infliximab ; macrolides (tacrolimus, pimecrolimus, sirolimus). Methods: Existing therapies: phototherapy (PUVA or UVB) for 4 weeks and for 6 weeks therapy: retinoids (acitretin), cyclosporin, methotrexate (MTX), and in very rare circumstances systemic steroids. Rotational therapy is necessary. Results: Topical therapy will continue to be used in combination with systemic psoriasis management for residual psoriatic plaques. We highlight the current psoriasis management, and their use in everyday practice in Croatia. Conclusion: The new agents may well transform the way that moderate to severe psoriasis is treated.
