Sažetak
At the University Hospital Merkur in Zagreb the orthotopic liver transplantations are routinely performing and 89 OLT has been performed. Six of them were performed as retransplantations. One year surviving is 85% and total surviving 83%, while the survival rate longer than one year is 75%. The indications for OLT were: Wilsons disease in 3 cases, carcinoma hepatocellulare, cirrhotic patients and in one patient hemangioendothelioma hepatis was present. Hepatitis C cirrhosis had 16 patients to whom OLT was performed, 9 patients had cryptogenic cyrhosis, 1 patient suffered from polycystic liver disease, primary sclerosans cholangitis was indication for OLT in cases, promary biliary cirrhosis and secondary biliary cirrhosis in 8 patients and 39 patients alcoholic and end stage liver disease. The incidence of the viral and fungal infections in liver transplanted patients is higher then in patients with other solid organs transplantations. The infections are associated by specific risk factors of recipients: pretransplant, transplant and posttransplant risk factors including liver disease. Species infections were indentified in 29 our transplant recipients. Three patients had systemic candida sp. infection, and in 22 patients candida sp. was in bile, wounds, drain or skin, 4 patients in urine. Most common candida albicans was isolated, while aspergillus species was isolated in transplant patients. Cytomegalovirus (CMV) infection was established in 15 liver recipients from whom only 1 had clinical symptoms. Two patients were infected with hepatitis B virus after othotopic liver transplantation. Presence of hepatitic C virus reinfection is diagnosed in the 15 patients transplanted for HCV cirrhosis, and 1 patient died because of fudjorant postransplan hepatits C virus chonic liver disease. One patient who was also transplanted because of HCV cirrhosis has developed PTDL (posttransplantation lymphoproliferative liver disease). The B cell nonHidgkin lymphoma was diagnosed which is mostly EBV virus donor derived.
