Sažetak
Regardless of the etiology of portal hypertension, clinical presentation is the same – esophageal varices bleeding, ascites, splenomegalia and hypersplenism. We would like to present our experience with portosystemic shunts in pediatric population. In the last three years we have performed 7 portosystemic shunts. Indications for the shunt procedures were biliary atresia (2), portal vein thrombosis (1), portal vein cavernoma (2), congenital hepatic fibrosis (1) and portal vein thrombosis after living donor segmental transplantation (1). We have used two shunt procedures, mesocaval "H" type shunt using vena iugularis interna graft in two cases and selective distal splenorenal shunt in five cases. The status of hypersplenism, liver and renal function and serum amonia level were parameters which we checked up regularily during follow-up period. One of our patients developed splenorenal shunt thrombosis so we performed splenectomy and esophagogastric devascularisation. Shunt procedure is effective and often definite therapy in patients with extrahepatic obstruction. In patients with intrinsic liver desease, shunt is effective procedure while waiting for final therapy – transplantation.
