Sažetak
Tissue hypoxia is a leading sign of septic shock. The oxygen uptake (VO2) is absolutely or relatively low. Production and elimination of CO2 (VCO2) is also impaired. We assumed that measurement of this phenomenon may be useful for evaluation of therapy success. We followed VCO2 in 18 adult patients with conventional criteria of septic shock during hemodynamic resuscitation. Resuscitation was guided after variables obtained by invasive hemodynamic monitoring (Swan-Ganz catheter). Colloid and cristalloid fluids as well as inotropes and vasopressors were administered when necessary. Initial findings indicated hypovolemic-hyperkinetic status with decreased VO2. VCO2 was measured by expired gas analysis via metabolic monitor. Initial findings after 3th and 6th. hour of therapy were taken into analysis. In seven survived patients baseline values of VCO2 were increased as compared in those who died (194 &plusmn ; ; 21 vs. 176 &plusmn ; ; 28, p<0.05 ml/min). VCO2 further increased in survivors, particularly after the 6th hour. In nonsurvivors, VCO2 did not change significantly despite improved hemodynamic profile. We consider that results of this preliminary study allow conclusion that simple measurement of VCO2 may provide useful information on severity of metabolic disorder in septic shock. The efficiency of resuscitation may be evaluated by this test.
