Sažetak
The aim of our study was to establish the connection between pain and depression, as well as pain and anxiety in children with JIA. The study subjects comprised 36 children with JIA, at the age from 8 to 16 years (median age 13, 3 years), 22 female (61, 1%) and 14 male (38, 9%), who all fulfilled the ILAR criteria for JIA. Disease duration was from 1 month to 14, 5 years (mean 50 months). Control group consisted of 35 health children, at the age from 7 to 16 years (median 12, 8 years), 20 female (57, 2%) and 15 (42, 8%) male. The pain was measured with visual analogue scale (VAS), which is considered to be a valid and reliable measurement for the intensity of pain and can be used in children above five years of age. The children were asked to mark the intensity of their pain on a 10 cm long horizontal line. The beginning of the scale signified no pain and the end the highest level of pain. For the measurement of depression and anxiety we used subscales used in Trauma symptom checklist for children (TSCC). TSCC consists of 54 statements, each marked on the 4-point scale from 0 to 3 (0-never, 1-rarely, 2-often, 3-always). The average selfperception of pain on the VAS scale was 29, 47 mm (SD: 19, 76, range 0-70 mm), The average selfperception of pain in girls was 31, 77mm and in boys 25, 85, which is not statistically significant. In children with JIA, the multiple regression analysis for pain predictor showed a statistically significant connection between pain and depression (p=0, 003) as well as between pain and anxiety (p=0, 039). Conclusion: It has been found that in children with juvenile idiopathic arthritis (JIA), joints inflammation can explained only a minor part of the illness, while the rest belongs to various psychosocial factors, all of which can influence their mood, affect and perception of pain, which is a clinically important symptom for a lot of these children. We can conclude that psychological testing can help professionals who work with chronic patients, especially with children who suffer severe pain. In this study we proved that children with JIA, who suffer severe pain are more depressed and have higher degree of anxiety. The treatment of children with juvenile idiopathic arthritis (JIA) is a very complex, and require constant co-operation of paediatricians and children psychiatrist
