PURPOSE: To report the management of late onset bleb leak with autologous blood injection in two eyes of two patients. METHODS: Two patients with thin, avascular blebs developed a late onset bleb leak three and four months after trabeculectomy with mitomycin C. Both patients were treated with a subconjunctival peribleb injection of approximately 0.2 ml of whole autologous blood. RESULTS: We recorded no intraoperative and postoperative complications in the first patient. After a follow up of 8 months the intraocular pressure ranged from 8 to 11 mmHg, and there were no signs of a recurrence of a bleb leak. In the second patient, due to hyphema formation, we encountered a transient IOP rise postoperatively, that resolved spontaneously. During a 6 month follow-up intraocular pressure ranged from 6 - 10 mmHg. CONCLUSIONS: Injection of autologous blood into a filtration bleb is a good treatment option in selected patients and it may avoid the need for surgical revision.
