Sažetak
This is a case reportpresenting hypersensitiviy to cow's milk in an exclusively breast-fed infant. The patient is a male, with a positive family history for atopy, and no prior illness. Symptoms began at the age of three months as feeding problems: he refused to suck at the breast and stopped thriving. The boy also refused infant formula, and perioral redess appeared following contact with formula. Diagnostic work-up revealed no sign of concomitant infection. Slightly elevated serum IgE levels were observed. The investigation of gastrointestinal tract showeddiffuse hystoligical hypersensetivity reaction, with eosinophils invading mucosa of the stomach, small intestine and colon (30 E/high power field). Symptoms disappeared and intestinal histology normalized when milk-free diet was given to breast feeding mother. The diagnosis of cow's milk allergy was challenged by exposure at the age of 8 months, when the mother sterted consumin dairy products. Hystology was again in the favour of eosinophilic colitis (20 Eo/HPF), but with no evident clinical consequences.Therefor the same dietary regime was continued and at the age of 10 monthsa new biopsy showednormal architecture and no tissue eosinophilia. At the age of 2 years boy eats dairy products and has no signs of allergy whatsoever. In conclusion, this boy was one of the rare patients reacting even to small cow+s milk protein conten in breast milk. Clinical tolerance proceeded histological recovery of gastrointestinal tract.
