Sažetak
Aim: The aim of the study was to histomorphometrically analyze the influence of overinstrumentation on the first stage of healing in chronic apical periodontitis in dogs. Methods: After inducing chronic apical periodontitis in 23 mandibular premolars (37 root canals) of six mongrel dogs, 17 root canals were instrumented and obturated to the apical delta (group 1). The length was confirmed radiographically. The other 20 root canals (group 2) were perforated apically and instrumented to the point determined electronically with resistance-type device EED 11 (Struja, Zagreb, Croatia) and obturated to the same lenght as in the group 1. The animals were sacrificed 35 days after the end of endodontic treatment. Undemineralized 5-7 µ ; m sections were stained with Toluidine-blue. The histomorphometric indices were measured by computer program ISSA (Vams, Zagreb, Croatia) under standard light microscope. Results: The differences between groups were statistically significant (p < 0.05 ; Mann-Whitney U test) in all the measured indices, except perimeter of the lesion (21.38 ± ; 6.86 mm group 1 ; 21.30 ± ; 4.82 mm group 2). Inflammatory cell count was higher in group 2 (111.39 ± ; 75.81 group 1 ; 137.62 ± ; 46.34 group 2) as well as the surface of the lesion (33.87 ± ; 2.40 mm2 group 1 ; 5.55 ± ; 3.96 mm2 group 2). Osteoid surface (10.34 ± ; 11.60 % group 1 ; 33.21 ± ; 21.43 % group 2) was greater in group 2. Osteoclast number (8.72 ± ; 10.69 group 1 ; 1.23 ± ; 1.92 group 2) and osteoclast index (111.34 ± ; 115.46 mm-2 group 1 ; 27.00 ± ; 39.03 mm-2 group 2) were lower in group were overinstrumentation was performed. Conclusion: Although inflammatory cell count and the lesion surface were higher in group 2, other histomorphometric parameters indicate that a higher healing potential was discovered when overinstrumentation was performed in the therapy of chronic apical periodontitis in dogs.
