Sažetak
The authors present an unusual case of a 30-year-old male patient with clinical signs of rhombencephalitis which had preceeded other clinical manifestations of neurosarcoidosis for several months. Paresis of cranial nerves N I, II, III, VII, VIII, IX, X and XII was found in the acute and subacute stage of the illness with peripheral nerve lesions in the chronic stage. Several cerebrospinal fluid routine examinations showed pleocytosis, increased protein content and decreased glucose content. Serum angiotensin converting enzyme (ACE) was elevated. Transbronchial biopsy of the lymph nodes was performed because of bilateral hilar lymphadenopathy and showed granulomatous inflammation without necrosis, as did the cytologic examination of skin biopsy. Muscle biopsy showed granulomatous inflammation without necrosis. The authors confirmed the disease after finding granulomatous inflammation without necrosis in the lymph nodes, as well as in the skin and muscle biopsy specimen, accompanied by elevated ACE. Clinical, diagnostic and therapeutic features are discussed.
