Sažetak
Pachyonychia - nail thickening is a constant symptom in Pachyonychia congenita (Syndroma Jadassohn-Lewandowsky), but it can be quite often present in several acquired diseases. Pachyonychia of some degree is present in about 5% of patients with severe psoriasis, mostly psoriatic erythrodermia, often in pityriasis rubra pilaris, and in some states with impaired circulation, advanced atherosclerosis or severe malapsorbtion in gastrointestinal polyposis. We observed therapeutical results of acitretin in both group od patients. In population of Croatia 25 patients with pacyonychia congenita were detected. Most of them in some period were trreated with acitretin. Certain success was achieved in palmoplantar keratodermia, follicular hyperkeratosis and leukokeratosis, but nails remained almost the same, somethimes extremely thickened and bizzare. On the contrary oral retinoids are often efficient on nail thickening in severe psoriasis. Improvements can be observed after six to eight months of treatment. Also in our two (out of seven) patients with pityriasis rubra pilaris, after six month of treatment with acitretin, erythrodermia disappeared and all yellow-brown thickened nails became normal. Beside of difference in origin, nail thickening in pachyonychia congenita is more resistant to therapy than in aquired diseases.
