Sažetak
Psoriasis is a multifactorial disease in which both genetic and environmental factors are responsible for the appearance and phenotypic expression of the disease. Psoriasis shows a polygenic pattern of inheritance. The key locus is located within the MCH region on chromosome 6p21.3. In terms of HLA associations, Hensler and Christophers have defined types I and II of psoriasis 1. The aims of the study were to assess the frequency of particular HLA alleles in 169 Croatian patients with type I and II psoriasis, and the extended risk haplotypes for type I psoriasis in the Croatian population. Control group included 190 blood donors. HLA class I and II genes were determined by polymerase chain reaction using allele specific oligonucleotide primers for specific sequences of particular HLA alleles (PCR-SSP). The ARLEQUIN software for population genetics was used on statistical analysis. In type I psoriasis, there was a significantly higher frequency of HLA alleles: B13 (odds ratio (OR)=8.89, P<0.001) ; B57 (=R=9.34, P<0.001) ; Cw*06 OR=12.78, P<0.001) ; DQB1*0701 (OR=9.70, P<0.001) ; DQB1*0201 (OR=4.92, P=<0.001) ; and DQB1*03032 (OR=53.98, P<0.001). In type II psoriasis, the frequeny of class I and II HLA alleles did not differ significantly from that recorded in the control group. Study results revealed the Croatian population of type I psoriasis patients to be characterized by two widely spread risk haplotypes:B13-Cw*06-DRB1*0701-DQA1*0201-DQB1*0202 (EH 13.1)and B57-Cw*06-DRB1*0701-DQA1*0201-DQB1*0303 (EH 57.1). A significant frequency of EH 57.1 haplotype was found in the group of patients with familial occurrence of the disease, and of EH 13.1 haplotype in those without familial clustering of the disease. The haplotypes recorded in patients with type II psoriasis did not differ significantly from those recorded in the control group. Conclusion: In type I psoriasis was found to be strongly associated with class I HLA alleles B13, B57, Cw*06 and class II alleles DQB1*0701, DQB1*0201 ans DQB1*0303. Extended haplotypes EH 13.1 and EH 57.1 are risk haplotypes for type I psoriasis in the Croatian population.
