Sažetak
Pemphigus vegetans is a rare variant of pemphigus with a characteristic localized vegetating papillomatous response. Two subtypes are described - pemphigus vegetans Neumann type and pemphigus vegetans Hallopeau type. We report on a 78-year-old female patient who presented to our Department with a yellowish-haemorrhagic crust in the medial part of the right eye and an erythematous patch covered with crust on the left preauricular region. A similar lesion was present in the presternal region as well as on the scalp. The lesions on the face were suspected as carcinomata and biopsy was performed that pointed to the diagnosis of vulgar pemphigus. On direct immunofluorescence there were intercellular C3 deposits and some granular deposits of IgM in the basement membrane zone. Indirect immunofluorescence was negative. Therapy with prednisolone was introduced in a dose of 1 mg/kg body weight daily (60 mg)together with local therapy. Dose tapering began after 3 weeks, and 6 weeks later there was almost complete regression of vegetating lesions. It is well known that lesions of pemphigus vegetans are mainly localized in the intertriginous areas (Neumann type), although the lesions in Hallopeau type and also be localized on the scalp and elsewhere. Differential diagnosis can sometimes be difficult as it was in our patient who was scheduled for operation before admission as a suspected carcinoma. In such cases experience along with laboratory testing is substantial to reach an accurate definite diagnosis.
