Sažetak
An analysis of the medicine consumption for ACT subgroups N 02 A and N 02 B from 1999 to 2002 in Croatia has been presented. The objectives of the investigation were to determine the rank of analgesics, which are most commonly used, and to determine whether the frequency of their usage is in accordance with the rational pharmacotherapy. Consumption was compared using the methodology of DDD and on the financial basis of wholesale patterns. The comparison was made by proprietary names, INNs as well as by rank and proportions. The first on the list of analgesics consumption throughout the whole period was the fixed-ratio combination of propyphenazone+paracetamol+caffeine+codeine (N 02 B), the next was tramadol (N 02 A), while acetylsalicylic acid, acetylsalicylic acid+vitamin C, paracetamol and metamizol (all N 02 B), were ranked third to the seventh position. Morphine (N 02 A) was most of the time financially at the tenth position. The increase of consumption of fentanyl in comparison with earlier period had also been noted. The usage of fixed-combination of analgesics with the same mechanism of action and different central effects is not justified from the standpoint of the rational pharmacotherapy. The rank of other generics of analgesics from N 02 B throughout the entire period was more or less the same and showed the rational admission in pain therapy. The exception is metamizole, which frequent usage is not justified due to its side effects. The high consumption of tramadol is probably connected to its parallel usage with analgesics from N 02 B. At the moment, the real reason behind the increase in fentanyl consumption in the recent period cannot be explained.
