Sažetak
Objective: The goal this study is to compare surgical outcomes obtained by small keyhole craniotomy approach with standard approach that includes larger craniotomy size. We wonder if minimally invasive keyhole concept can be good neurosurgical alternative to endovascular treatment in terms of outcome. Patients and Methods: Out of 562 patients with intracranial aneurysms in 7, 5 years period, 427 patient was operated upon through small keyhole craniotomy but by large eyebrow-keyhole craniotomy was used. The group of patients operated upon a standard larger craniotomy consisted of 135 patients. Both groups were similarly mached according to the Hunt-Hess scale, and Glasgow outcome scale. Results: Comparison of surgical outcomes for both groups gave a similar result. We have found excellent and very good outcome (GOS 5 and 4) in 351 (82, 21%) patients from keyhole craniotomy group, and in standard craniotomy group the same outcome was found in 109 patients (80, 74%). The mortality rate was 0, 93% for keyhole group of patients and 2, 22% for standard craniotomy group. Conclusions: Our study has shown no statistical difference in outcome for both of groups. Results are especially good for keyhole group of patients. We think that minimally invasive concept can be even a very good alternative to endovascular treatment especially when cost benefit analysis is concerned.
