Sažetak
High-energy ultrasound is commonly used today in brain operations. Cavitation Ultrasonic Surgical Aspirator (CUSA) is used to destroy malignant tumor tissue. Its radiation pattern is practically omnidirectional and radiates high intensity ultrasound in the tissue. The ultrasound is reflected from bones and areas in human brain with unwanted increased ultrasound energy are presented. The use of computer simulation enables real situations during neurosurgical operations by CUSA to simulate. Ultrasound probe with improved properties is being developed. It is called Neurosurgical Contact Ultrasonic Probe. The construction is based on the transformation of electrical energy by composite piezoelectric ultrasonic transducer into very strong mechanical longitudinal vibrations of elastic titanium wire of 1.6 mm to 2 mm in diameter and 350 mm to 900 mm long. The optimal electroacoustic performance of this complex construction of ultrasonic transducer has been achieved by selection of appropriate dimensions, shape, elastic properties and density of materials. These elements are also essential in selection of intrinsic mechanical resonant frequency of the composite piezoelectric transducer. This frequency is approximately 24500 Hz. The probe with titanium wire ensures sufficient mechanical energy for fragmentation of firm cystic products (characterized by acceleration, vibration velocity and probe tip displacement amplitude). The primary objective of the construction of the endoscopic ultrasound probe is to simplify and improve surgical procedures in the treatement of hydrocephalus and certain types of cystic and tumor diseases in difficult to access but functionally important regions of the neural tissue. Due to contact access and hammer effect the operation is fully controled, so the influence of ultrasound field on the neighboring healthy tissue is negligible. The operative procedure was tested on 240 laboratory animals and then confirmed in vivo in more then 60 surgeries on humans (hydrocephalus and tumor). Development of fully noninvasive method is based on developing an array of ultrasound transducers to accomplish the focusing of ultrasound energy into a single point in brain tissue.
