Sažetak
The background of this overview is more than 30 years experience of the senior author in thyroid surgery based upon more than 6500 thyroidectomies, more than 1200 performed for malignant tumors.Beginning our work in early 70's we have accepted following strategy in thyroid surgery: total lobectomy as a basic procedure for thyroid node(s), total thyroidectomy for all differentiated and medullary carcinomas, paraglandular dissection and/or lateral neck dissection for metastases in central region of the neck and lateral regions respectively. Since June 1998 to June 2002 at our department total of 1127 thyroid surgical procedures were performed ; for benign goiters 771(68, 4%), for primary malignant tumors 314(27, 9%) and for secondary malignancies 42(3, 7%).Out of benign goiters follicular adenomas in 59, 9%, Huerthle cell adenomas in 3, 9%, nodular form of autoimmune thyroiditis in 1, 3%, polynodular goiter in 15, 8%, more than a half with a substernal extension, in 14% Graves disease and in 5, 1% polynodular toxic goiter were found. Out of primary malignancies there were 260(83, 2%) papillary(PTC), 38/12, 3%) follicular(FTC), 11(3, 6%) medullary(MTC) and 2(0, 9%) anaplastic cancers(ATC). Male/female ratio was 1:6.6 for benign and 1:5.1 for malignant goiters.Mean age for PTC was 47, 9ys, for FTC 48, 9ys, for MTC 50, 7ys and for ATC 79ys. The smallest tumor was 0, 12 cm and the largest was 10, 0 cm in diameter. Mean diameter for PTC was 1.8 , FTC 3.2, MTC 2.3 and ATC 8.7 cm respectively.For differentiated tumors under 45ys of age there were 119 in stage I and 2 in stage II, and over 45ys 47 in stage I, 94 stage II, 28 stage III and 1 in stage IV.
