Sažetak
Cerebrovascular events in children, half of which are ischemic strokes, occur at an estimated incidence of about 2 per 100 000 per year. In approximately one third of cases no obvious cause or underlying disorder can be detected. Elevated lipoprotein (a), Lp(a), has been identified as a genetically determined risk factor for stroke in young adults, but only preliminary data are available on its role as a risk factor for ischemic stroke in infants and children. In order to increase the knowledge about the etiology of cerebrovascular disease in childhood, we studied the prevalence of elevated Lp(a) in 27 children (15 males and 12 females ; age range 1-15 years) with a history of arterial ischemic stroke (AIS, N=8), transient ischemic attack (TIA, N=8) and epilepsy (EPI, N=11) as well as 15 control subjects (8 males and 7 females ; age range 2-17 years). Diagnoses of TIA, AIS and EPI were confirmed by patients' history, clinical signs and symptoms and neurologic examination including computed tomography and magnetic resonance. A significantly higher frequency of Lp(a) values above 0.30 g/L were found in patients (7 of 27 ; 26%) compared with control subjects (1 of 15 ; 7%), p<0.001. Among children with a history of AIS, 4 of 8 children had significantly higher concentrations of Lp(a) (median 0.44, range 0.04 - 1.92) compared with controls (median 0.08, range 0.04 -1.45). In conclusion, despite a low number of children in each group, we believe that individuals with genetic predisposition for higher Lp(a) values seem to have important risk factor for cerebrovascular disease already in childhood
