Background and Aims: The patient empowerment approach to diabetes care is being increasingly recommended. This study was aimed at determining impacts of an empowerment-based psychosocial intervetion on the patients'quality of life and glycemic control as compared to patients in standard care.Materials and Methods:Type 2 patients consecutively recruited among patients scheduled for their regular medical check-ups were individually acquainted with empowerment-based principles and invited to participate in an empowering educational course. A response rate was 35% giving a number of 73 patients who were treated in 8 separate groups.The participating patients were 53.3 (5.1) years old, had 10.4 (3.0) years of education, suffered from diabetes for 8.0 (5.6) years, were orally treated in 80% cases, with baseline HbA1C of 9.65% (1.43%), diabetes complications in 50% and concomitant siseases in 73% cases.Health beliefs and personal model of diabetes were assessed at recruitment, while quality of life was assesed prior to and after the course.Glycemic control was measured prior to, after the course and after 3- and 6-month follow-up periods.The measures of quality of life and glycemic control were compared with those of the patients treated in standard care (N=35). Demographic and disease-related characteristics, as well as baseline quality of life, health beliefs and personal model of diabetes did not differ between patients from the intervention and the control group. The programme was structured according to C. Feste including 6 weekly sessions referring to goal setting, problem solving, coping with diabetes, coping with daily stress, seeking social support and staying motivated. Results: The treated patients reported their quality of life to be improved after the course regarding psychological and social aspects (both p<0.0001). Their glycemic control also improved and remained so after 3- and 6-month follow-ups, still being in a category of poor control (HbA1C decreased from 9.65% to 8.96 % after 6 months p<0.05). Regression analysis indicated that educational level, beliefs supporting internal health control, and treatment efficacy beliefs may predict glycemic control at follow-up periods (standardized Betas: -0.18 ; -0.12 and -0.21 ; all p<0.05). No changes regarding quality of life and glycemic control were registered in the control group (all p> 0.05). Conclusion: Empowerment-based psychosocial intervention in type 2 diabetics affects their quality of life favoraubly and has positive, but limited effects on glycemic control. Better educated patients believing in internal health control, as well as in efficacy of diabetes treatment seem to benefit the most.
