Sažetak
Acute lower respiratory infections (ALRI) in children under five are still the leading cause of childhood mortality in the world. The WHO recently estimated the annual number of ALRI-related deaths at 2.1 million. In this paper we aim to estimate the incidence of ALRI in young children in developing countries, which is a close approximation of the global ALRI incidence. Our estimate is based on an analysis of published data on the ALRI incidence from community-based longitudinal studies. Among more than 2, 000 studies published since 1961, we identified 46 studies reporting ALRI incidence and 28 met pre-defined quality criteria. The median incidence estimate from those studies was 0.28 episodes per child-year (25% - 75% inter-quartile range 0.21-0.71). We assessed the plausibility of this estimate in two ways. The first approach was based on global ALRI mortality and reported case-fatality rates for (all) ALRI reported in community-based studies. This yielded a median incidence of 0.20 episodes per child-year (25% - 75% inter-quartile range 0.15-0.31). The second approach was based on global ALRI mortality, reported case fatality rate for severe cases only and estimated proportion of severe cases among all ALRI cases. This produced a median incidence of 0.44 episodes per child-year (25% - 75% inter-quartile range 0.28-0.76). These analyses suggest that the global ALRI incidence is close to 0.29 episodes per child-year in children under 5 years of age in developing countries. This equates to an annual incidence of 154 million new ALRI cases, 11-20 million (7-13%) of which are severe enough to require hospital admission.
