Sažetak
In hepatitis B viral (HBV) infection (and similar is in HDV, HAV and HEV infection) it is possible to determ HBV surface antigen (HbsAg) what enables important diagnostic and prognostic information about hepatitis B. In hepatitis C virus (HCV) infection, howewer, until recently it was not possible to determine specific antigen &#8211 ; HCVAg. Introduction of HCVAg determination in their free (during 2000) and bound (during 2002) form open quite new possibilities in diagnosis, prognosis, transmission prevention, monitoring of therapy eficiency and generaly about insight in natural history of HCV infection. Namely, analysis of free HCVAg essentialy facilitate , in adequate circumstances, establishing of diagnosis of acute HCV infection and determination of (relativlely) precise time of virus entry into organism. On other side it was shown that bound HCVAg ( in complex with specific antibody) is in very good quantitative correlation with HCV-RNA. Through this way, among others, it opens possibilities of relatively fast, cheap and simple determination of HCV status of anti-HCV positive individuals and HCV therapy efficiency monitoring and their partly substitution from relativly still expensive, complex and with certain problems charged PCR procedure. In this study free and bound HCVAg were analysed through the technic of enzymatic immunologic procedure on the following groups of examinees: 1. patients with acute nonA-nonC hepatitis viral infection ; and 2. patients with chronic HCV infection. Results have shown that among individuals with acute nonA-nonC infection, which will thereafter, in variable period of times, develop anti-HCV, free HCVAg is present in 98% of case up to 2, 5 months before appearing of specific antibody (anti-HCV). In patients with chronic HCV infection there exists a different level of bound HCVAg in high correlation with HCV-RNA. It could be concluded that HCVAg analysis essentialy facilitates diagnosis of acute HCV infection and resolves some important dilemmas in chronic HCV infection.
