Sažetak
New model of providing comprehensive care (pharmaceutical care practice) where the pharmacist works directly with the patient in collaboration with physicians and other rmembers of the multidisciplinary health care team to optimize patients' medication experience and clinical outcomes. Pharmacist’s goal is to understand patient’s unique medication experience-be his partner, help them make best decisions regarding their therapy. The care process becomes specific to pharmacy when it is driven by a philosophy of practice and supported by a practice management system where addressing all of a patient’s medication related needs is the central purpose of the pharmacist’s work.
