Sažetak
Objective. To assess the effectiveness of 3-month treatment with 5 mg ulipristal acetate (UPA) on uterine fibroid volume reduction and improvement of quality of life in affected reproductive age women. Study design. This observational study included twenty women of reproductive age requiring surgical fibroid treatment. Patients were treated before surgery with UPA 5 mg for 3 months. The change of total fibroid volume for each patient was calculated. The Uterine Fibroid Symptom and Quality of Life questionnaire (UFA-QoL) was given to each participant before and after UPA therapy. Results. Fifteen patients completed the study. Four patients (26, 7%) had an excellent response to therapy (>50% reduction). Six patients (40%) had a good response (25-50% reduction) while five patients (33, 3%) experienced no clinically relevant response (<20%). The total fibroid volume after UPA treatment were between 2, 30 cm³ and 317, 33 cm³ with median of 58, 73 cm³. The overall severity of the symptoms score before treatment initiation was 57, 9±14, 7. Fibroid related symptoms were significantly reduced after 13 weeks of UPA treatment 25, 46±14, 73 and this was not related to reduction of fibroid volume. Reduced fibroid symptom severity following UPA treatment resulted in significantly improved USF-QoL in our studied population (total score of 90, 00±14, 73). Conclusion. In this study we confirmed the therapeutic efficacy of ulipristal acetate in patients with uterine fibroids. This finding brings many benefits to the patients, especially considering the preservation of body integrity and reproductive potential in women of childbearing age, delaying or avoiding surgical treatment in patients of premenopausal age and improvement of quality of life in all groups of patients
