Sažetak
Wernicke-Korsakoff syndrome (WKS) occurs in 2% of alcohol use disorder (AUD) patients and includes two entities. The first is Wernicke encephalopathy (WE), and subsequently, if WE is not treated, more than 50% of all cases progress to Korsakow syndrome (KS) which can result in permanent neurological deficits and death. The syndrome is characterized by a wide range of symptoms but the most specific are anterograde and retrograde amnesia. Unfortunately, it still often remains unrecognized and untreated. A 66-year-old female university professor with a history of alcohol misuse. was admitted to the University Psychiatric Hospital Vrapče, due to her incapability to abstain from alcohol on her own, along with a rather depressed mood, hipobulia, and a feeling of confusion. Her first admission was followed by a head contusion sustained in an intoxicated state. The regular MSCT scan showed no acute pathomorphological substrate, as well as normal intracranial anatomy in general. She was normally oriented, with psychomotor retardation, cooperative but prone to confabulation. Along with impaired mnestic functions, her cognitive capacity also appeared to have deteriorated. The neuropsychological evaluation showed average intellectual functioning with clear predomination of organic cerebral dysfunction and poor results on memory tasks. High doses of the vitamin B complex (especially thiamin) were administered. Although the high doses of the vitamin B complex were administered, she remained undercapacitated in forming novel comprehensive memories, with little insight into the condition mitigating it with fragmented confabulations. Due to significant cognitive impairment she could not participate in the treatment program. The patient had developed a full Korsakow’s syndrome without previously recognized WE, due to excessive alcohol consumption. Her mental state has not improved much since her first admission, despite the adequate doses of vitamin B which is considered as main therapy along with abstinence. Abstinence is necessary regardless of the administration of vitamins, which the patient did not maintain. Most patients with KS improve their cognition after establishing abstinence, but 25% require long-term rehabilitation. According to established therapeutic guidelines, WKS is entirely preventable by early recognition and treatment of high-risk AUD patients with parenteral and oral thiamine.
