Sažetak
We read with interest the article by Waldo and coauthors1 on the impact of surgical ineligibility on mortality after percutaneous coronary interventions among patients with left main or multivessel coronary artery disease. Briefly, the authors found that coronary artery bypass grafting (CABG) turndown created a treatment bias with an adverse impact on long-term mortality after percutaneous coronary intervention. We believe that present-day rationale for surgical ineligibility may be driven by the fact that CABG mortality remains an important cardiac surgical metric of excellence. The fact, however, that Waldo et al found that 22% of patients with complex coronary artery disease would be deemed inoperable should raise alarm in the surgical community.
