Sažetak
Rationale: More than 50% of patients with biliary obstruction are malnourished, but the mechanisms involved are not well understood. The aim of this study was to determine the influence of obstructive jaundice on the hormones controlling appetite and nutritive status of patients before and after endoscopic internal biliary drainage. Methods: 55 patients with obstructive jaundice were prospectively investigated, 34 with benign and 21 with malignant disease, along with 40 non-jaundiced controls. Appetite, nutritional status, serum ghrelin, cholecystokinin, IL-6 and TNF-α were determined at admission, 48 hours and 28 days after internal biliary drainage. Results: Patients with obstructive jaundice were significantly malnourished compared to controls, regardless of disease etiology. Plasma ghrelin but not cholecystokinin levels were significantly higher in patients with obstructive jaundice [median 1549 (1039, 0-2404, 0) vs. 533 (368, 5-931, 5) pg/mL, p<0, 001]. Plasma ghrelin levels were negatively correlated with CRP (rs= -0, 370, p=0, 005). Serum bilirubin concentration was negatively correlated with ghrelin levels (rs = -0, 424 ; p=0, 001), positively correlated with TNF-a (rs = 0, 486 ; p<0, 001), but no correlation was established for cholecystokinin concentrations. After internal biliary drainage, a significant improvement of nutritional status was observed in spite of the fact that serum concentrations of ghrelin, IL-6 and TNF- α remained significantly elevated even 28 days after the procedure. We have not established correlation between appetite and serum levels of ghrelin, cholecystokinin, IL-6 and TNF- α before and after biliary drainage.
