Background: Successful deprescribing often involves a collaboration between a pharmacist and a physician. Sharing agreement on a deprescribing target is an important foundation. Pharmacist’s rationale behind a recommendation can help physicians in decision-making. Objective: The aim of this study is to explore the level of agreement on potential deprescribing between pharmacists and physicians. Methods: Cross-sectional survey on pharmacists' and physicians' agreement on therapy requiring deprescribing in Croatia.Case-vignette of a multimorbid elderly patient using 15 medicines with high willingness to have medicine deprescribed, developed by a clinical pharmacist specialist and a community pharmacist. Pharmacists were asked for deprescribing suggestion.Physicians were asked to state agreement with the proposed suggestion.Survey was distributed via email through healthcare providers’ associations Results: A total of 89 pharmacists and 83 physicians participated. Both pharmacists and physicians were most willing to deprescribe NSAID, opioid and diuretics (84.30%, 83.72% and 71.50% participants), and were least willing to deprescribe solifenacin and vitamin supplement (37.79% and 36.05% participants). Pharmacists suggested stopping diuretic and opioid due to side effects (38.20% and 37.08%), tapering benzodiazepine (29.21%), and stopping NSAID and ASA due to lack of appropriate indication (23.59% and 25.84%). The least rationales were given for stopping allopurinol (5.62%) and reducing the dose of solifenacin (4.49%). Physicians were more likely to deprescribe all medicines than pharmacists (p<0.005), except for NSAID for which they equally agreed (χ2 (1) =2.86 ; p=0.091). Conclusion: Physicians agree with pharmacist’s deprescribing suggestions indicating a potential collaboration is possible. Pharmacists show reluctance towards offering deprescribing suggestions
