Sažetak
Tinea capitis is caused by two genera of dermatophyte fungi. Trichophyton and Microsporum, the latter more difficult to treat because of its ectothrix nature. Griseofulvin, the standard therapy, requires treatment durations of 4-8 weeks or longer. Terbinafine has fungicidal activity against dermatophytes and persistence in hair and skin, properties that might permit shorter treatment durations, an important consideration in children, the group most at risk for tinea capitis infection. A randomized Phase II duration-finding was carried out in 177 patients (mean age 7-4 years) with Trichophyton tinea capitis. Complete cure (negative culture plus absence of any signs and symptoms) was attained in 42%patients treated for 1 week, 49% for 2 weeks and 56% for 4 weeks. In a second Phase II stady, 165 patients ( mean age 7, 7 years) with Microsporum infections were randomized to 6, 8, 10 or 12 weeks of terbinafine, or to 12 weeks of high-dose griseofulvin (20mg/kg/day) treatment. The high dose griseofulvin group was included to determine the maximum effect prolonged high-dose administration of the standard therapy. For terbinafine, the complete cure rate was 62% after 6 weeks and did not increase with longer treatment. In the high-dose griseofulvin group, complete cure was achieved in 84% patients. In both studies, terbinafine was safe and well tolerated. At the dose of terbinafine used in these studies 2-4 weeks of treatment appears to be efficacious in tinea capitis caused by Trichophyton species and 6 weeks for the majority of patients with Microsporum canis.
