Sažetak
A double-blind clinical trial was conducted to compare the efficacy and safety of ampicillin and gentamicin in patients with hepatic encephalopathy. The study included 31 patients of both sexes with grade 0-III. hepatic encephalopathy. Seventeen patients were randomly assigned to ampicillin 1500 mg (group 1) or gentamicin 240 mg (group 2) per day, divided in three doses. At the beginning and at the end of the 7-day study, serum ammonia and potassium were measured, grade of hepatic encephalopathy and possible improvement were evaluated, and 'number connection test' was conducted. Serum ammonia decreased in group 1 from 79.6 ± 29.3 to 57.9 ± 19.1 μmol/L ; p = 0.007, and in group 2 from 85.8 ± 16.1 to 75.2 ± 45.1 μmol/L ; p = 0.15. There was a slight but clinically irrelevant increase in the serum potassium level. The connection test time was significantly reduced in both groups (in group 1 from 127.9 ± 46.9 to 92.5 ± 42.1 s or by 28% ; p = 0.0027, and in group 2 from 105.0 ± 52.4 to 61.2 ± 20.9 s or by 42% ; p = 0.0033). The patient mental status score decreased in 14 group 1 (82%) and in 11 group 2 (78%) subjects. Although ampicillin had a more pronounced effect on ammoniemia, and gentamycin on the connection test results, the observed differences did not reach statistical significance. It is concluded that both antibiotics under study showed comparable efficacy and torelability in the treatment of hepatic encephalopathy, ampicillin being less expensive, has lower toxicity, and shows some advantages on parenteral use.
