Highly active antiretroviral therapy (HAART) has a great impact on morbidity and mortality of patients infected with the human immunodeficiency virus (HIV). However, HAART is associated with acute and long term adverse events. The aim of the study was to assess dermatologic adverse events observed in patients treated at the University Hospital of Infectious Diseases, Zagreb, Croatia, during the 1998-2001 period. Cutaneous adverse events can be classified into events related to immune reconstruction such as herpes zoster ; adverse effects of drugs such as rash ; and retinoid effect of some antiretroviral medications. Adverse events are related to different classes of drugs, e.g., mononucleoside analogues are known to cause rash, and cases of Stevens-Johnson syndrome have also been reported. Skin retinoid effects are related mainly to the protease inhibitor indinavir. We report on a patient who developed thickening of almost all fingers after 27-month treatment with indinavir (800 mg tid), zidovudine and lamivudine. New periosteal bone formations and osteosclerosis of the spine were observed. It was explained by the retionid effect of indinavir, as the patient also had dry skin, ingrown toenails, and hair loss. Skin and nail adverse events significantly compromise the well being of patients treated with HAART, however, they are often neglected.
