AIM: Odontomas are one of the most common benign odontogenic tumors formed from all odontogenic tissues (enamel, dentin, cement and pulp tissue). They are divided into two types: complex and compound odontomas with characteristic clinical, morphological, radiographic and histological features. The etiology of the odontoma is poorly known, although some studies have identified local trauma, inflammatory and chronic infections during odontogenesis, and genetic mutations as possible factors. Although most odontomas are often associated with disorders of tooth eruption, causing impaction and/or delayed tooth eruption and malpositions, the majority of odontomas are asymptomatic and are discovered as an accidental finding during routine radiographic examination during the first two decades of life regardless of gender. CASE DESCRIPTION: We present a rare case of a multiple compound odontoma in an 11-year-old boy which was causing impaction of the first lower right permanent molar, 46. A Computer Tomography showed the presence of two irregular radiopaque amorphous masses of approximately 14.7 x 12.82 mm, located above each other and above the crown of the impacted tooth 46 and surrounded by a radiolucent cavity separating it from the surrounding bone and from each other. Treatment consisted of surgical enucleation of the odontomas with preservation of the impacted tooth. After 6 months, the clinical evaluation revealed the spontaneous eruption of the impacted tooth 46 based on the new radiographic examination and was concluded that the impacted tooth was derotated. Therefore, orthodontic treatment was proposed after spontaneous eruption of the impacted tooth to achieve an acceptable occlusion. CONCLUSION: Odontomas are not an everyday part of the clinical practice, therefore it is extremely important to have knowledge of their clinical and radiological features. Since odontomas are most commonly associated with permanent tooth impaction, early diagnosis and adequate treatment results in a better prognosis of preserving the impacted teeth.
